Roles & Responsibilities
- Provide crisis intervention and counseling to patients in acute settings
- Conduct comprehensive assessments and develop individualized care plans
- Collaborate closely with interdisciplinary teams to coordinate care
- Document all interventions and progress in EPIC charting system
- Participate in mobile crisis outreach to support patients in the community
- Maintain a consistent 40‑hour weekly schedule with 80 hours every two weeks
- Respond to on‑call duties as needed to ensure continuous patient support
Qualifications
- Licensed Clinical Social Worker (LCSW) with at least 2 years of experience
- BLS (AHA) certification
- Proficiency in EPIC charting system
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Commitment to providing compassionate, evidence‑based care
