Must Have Hospice Staffing/Scheduling Experience! Hospice Staffing Coordinator Join our dedicated team at Affinity Care of NJ as a Hospice Staffing Coordinator in Brick Township, NJ ! We are committed to providing compassionate care and support to terminally ill patients and their families. About the Role: As a Hospice Staffing Coordinator, you will play a vital role in ensuring that our patients receive the highest level of care and support. Your responsibilities will include: Coordinating patient admissions, discharges, and transfers to ensure seamless care transitions Maintaining accurate patient records, medical charts, and electronic health records Collaborating with interdisciplinary teams, including physicians, nurses, and social workers, to develop and implement patient care plans Communicating effectively with patients, families, and healthcare providers to ensure optimal care and support Identifying and addressing patient care gaps and trends to improve outcomes and quality of care What We Offer: A dynamic and supportive work environment Opportunities for professional growth and development A chance to make a meaningful difference in the lives of our patients and families Requirements: 2 years of experience (recent graduates and transitioning professionals welcome!)
