Wound, Ostomy, and Continence Nurse

Schedule: 7:30 AM - 4:00 PM

Job Description

The Wound, Ostomy, and Continence Nurse provides patient care based on nursing guidelines and applicable standards of care. This role serves as a liaison for wound, ostomy, and continence care concerns and provides education to team members while participating in assigned committees.

Responsibilities

• Assess the physical, psychological, and social needs of assigned patients.
• Initiate data collection and assessment on assigned patients.
• Contribute to the development of the plan of care for each assigned patient in collaboration with the interdisciplinary care team.
• Implement nursing interventions, medical orders, and related therapies.
• Collaborate with the healthcare team to educate patients and their support systems.
• Analyze, modify, and implement strategies to progress patients through their care plan.
• Document patient records reflective of provided care and patients' responses in accordance with professional standards.
• Monitor equipment and perform routine and complex treatments including ostomy care, wound care with draining of wounds and fistulas, and managing fecal and urinary incontinence care.
• Comply with mandatory reporting, legal consent, transfer protocols, and other requirements that preserve patient rights.
• Provide a clean, safe, and secure environment for patients, following safety policies and procedures.
• Handle biomedical, pharmaceutical, hazardous, and universal wastes per regulatory standards, and if assigned, demonstrate safe handling of hazardous drugs.

Required Experience / Certifications / Licensure

• Inpatient acute wound care experience required.
• Experience in continence care and ostomy care is required.
