Trauma-Informed Community Case Manager

Tellurian, Inc in Madison, Wisconsin is seeking a Clinical Case Manager to work with the Community Intervention Team. This role focuses on supporting adults with mental health and substance use disorders through community resources and tailored case management. The successful candidate will engage clients, conduct assessments, and collaborate within a multi-disciplinary team. A Master's Degree in health/social services is required along with 2 years of experience in the Human Services field.
