Job Description Provide direct patient care in a home setting, ensuring the comfort and well-being of patients Administer medications and treatments as prescribed by the physician (Wound Vac, IV Certified, Wound care, etc.) Monitor and record vital signs, such as blood pressure, temperature, and heart rate Implements plan of care initiated by the registered nurse / referring doctor Educate patients and their families on proper healthcare procedures and disease management Provides accurate and timely documentation consistent with the plan of care Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor Participates in personal and professional growth and development Uses equipment and supplies effectively and efficiently The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
