CHN is currently assisting a growing home care agency with a need for an experienced Clinical Manager. Position Overview A growing home care agency in the Needham, MA area is seeking an experienced Clinical Nurse Manager (RN) to lead and support clinical operations. This role is ideal for a hands‑on leader who thrives in a fast‑paced environment and is passionate about delivering high‑quality, patient‑centered care in the home health setting. The Clinical Nurse Manager will oversee clinical staff, ensure compliance with Medicare‑certified home health regulations, and drive excellence in patient outcomes, staff performance, and operational efficiency. Key Responsibilities Provide day‑to‑day clinical leadership and supervision of field nurses and clinical staff Ensure compliance with Medicare Conditions of Participation (CoPs) and all state/federal regulations Oversee clinical documentation, OASIS accuracy, and care plan development Conduct case conferences, chart audits, and quality assurance reviews Collaborate with intake, scheduling, and administrative teams to ensure seamless patient care delivery Support staff onboarding, training, and ongoing performance management Serve as a clinical resource and mentor to nursing staff in the field Monitor and improve patient outcomes, satisfaction, and quality metrics Participate in survey readiness and accreditation processes Assist with growth initiatives, including program development and service expansion Qualifications Active Registered Nurse (RN) license in Massachusetts (required) #J-18808-Ljbffr Community Health Network, Inc.
