A-Line Staffing is now hiring Home Care RN in Union / Somerset County. The Home Care RN would be working for a Major Healthcare System and has career growth potential. Home Care RN Highlights: Schedule: Monday – Friday, 8:00 AM – 4:30 PM Pay Range: $60-$70 hourly, plus mileage reimbursement Home Care RN Responsibilities: Assess home health/hospice patients in stable and emergent situations to determine physical, psychosocial, and environmental needs. Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Incorporate physical, psychological, social, spiritual, and cultural factors into the POC. Maintain up-to-date patient records with accurate documentation. Provide care using infection control measures (OSHA guidelines). Educate patients and families on care, diet, medications, and home safety. Supervise and provide clinical direction to HHA/STNA/LVN staff. Assess and manage pain using appropriate pain management techniques. Arrange referrals to external agencies when necessary. Monitor compliance with third-party payer requirements. Provide on-call services as assigned. Communicate effectively with patients, families, physicians, and healthcare professionals. Attend interdisciplinary team meetings and other required meetings. Perform other duties as assigned. Home Care RN Required Qualifications: Current Registered Nurse (RN) license Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program Proof of CPR certification Current driver’s license & reliable transportation with insurance 1-2 years of clinical RN experience 2+ years of home health experience Homecare Homebase Software and/or OASIS Experience – highly preferred Preferred Qualifications: CHPN certification Bachelor of Science in Nursing (BSN) If you are interested in this Home Care RN position, please apply to this posting with Luke Heuser at A-Line! #J-18808-Ljbffr
