St. Joseph's Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a community-based Intermediate Care Facility home in Hughestown, PA. As an LPN at St. Joseph's Center, you'll play a vital role in ensuring the health and well-being of our residents. Provide direct nursing care and health monitoring for residents. Administer medications accurately and monitor for side effects. Develop, implement, and evaluate individualized care plans. Maintain medical records and ensure compliance with ICF/ID regulations. Assist residents with activities of daily living (hygiene, dressing, feeding). Accompany residents to medical and dental appointments. Respond to health emergencies and implement appropriate interventions. Collaborate with the interdisciplinary team and participate in care planning. Maintain adequate medical supplies and equipment. Support residents in community activities and outings.
