Travel Physical Therapist - SNF / LTC - $2,238 per week

Job Overview Centra Healthcare Allied seeks a travel Physical Therapist for a 13‑week assignment in Largo, Florida. The position is with a highly regarded Skilled Nursing Facility/LTC facility looking for an experienced or newly graduated PT to join their growing team. Responsibilities Assess referred patients requiring rehabilitation services, determining medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Provide direct patient care and establish a treatment program with specific goals based on patient capacity and tolerance. Set realistic and achievable goals, document and record patient condition, and educate patients and families in the appropriate physical therapy methods. Qualifications Graduated from an accredited school. Current state license as a Registered Physical Therapist or actively in the process of obtaining one. Current CPR certification. Develop and implement a plan of care for each patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
