Role Overview: As a Home and Community Based Services Licensed Practical Nurse, you will deliver direct patient care in patients' homes, following physician prescriptions. You will document clinical notes, coordinate care activities, and ensure high-quality services within the scope of nursing practice. Key Responsibilities Provide clinical services according to state laws, care plans, and infection control protocols. Thoroughly document patient visits in electronic medical records and visit verification systems. Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, clinicians, and team members. Adapt to diverse home environments, respecting clients' backgrounds and conditions. Qualifications & Skills Unrestricted LPN licensure in the practice state. Valid driver's license, reliable transportation or access to public transit. Preferred: CPR certification, flexibility, strong communication, organizational, and multitasking skills, and ability to work independently.
