Care Coordination Specialist
Via Care Community Health Center
Via Care Community Health Center is seeking a Case Manager to help patients understand health care recommendations, perform basic patient care tasks, screen medical calls, and document information in medical records to support treatment planning. You will coordinate referrals, arrange follow-up appointments, provide health education, and assist with translation and back-office tasks while upholding HIPAA and care standards in a fast-paced clinic. #J-18808-Ljbffr
