Social Services Director
Job Summary:
Prairie Oasis is seeking an experienced and compassionate Social Services Director to join our team in South Holland, Illinois . As a key member of our multidisciplinary care team, you will be responsible for providing exceptional social services to our residents and ensuring a seamless transition from hospital to home. Responsibilities:
Lead and manage the social services department, overseeing daily operations and ensuring compliance with regulatory requirements and company policies
Conduct comprehensive assessments to identify residents' social, emotional, and psychological needs, and develop individualized care plans to address those needs
Collaborate with care teams, healthcare professionals, and family members to ensure a coordinated approach to care and effective communication
Coordinate and facilitate discharge planning, ensuring a smooth transition from hospital to home or alternative care settings
Develop and maintain relationships with community organizations, home health agencies, and other healthcare providers to ensure access to necessary resources and services
Monitored residents' progress and adjust care plans as needed to achieve optimal health, physical function, and independence

Requirements:
Master's degree in social work or related field
Previous experience in Nursing Home Facility/Long Term Care
