Home Health Registered Nurse

We are seeking a dedicated Registered Nurse to provide skilled home health care across a variety of patient needs. This role focuses on delivering high-quality clinical services, including admissions and ongoing patient care management, while ensuring accurate documentation and collaboration with the care team. The ideal candidate is adaptable and comfortable working a flexible schedule that may include days, evenings, weekends, and on‑call rotations. Schedule & Work Details Schedule: Flexible shifts including days, evenings, and weekends Weekly Guarantee: 36 hours Productivity Expectation: 30 points On-Call/Weekend Requirement: Every 3 to 4 weeks when fully staffed Mileage Reimbursement: Not provided Pay Rate: $51 to $53 per hour Key Responsibilities Perform Start of Care (SOC), Resumption of Care (ROC), and recertification visits Conduct comprehensive patient assessments and develop individualized plans of care Monitor patient conditions and update care plans as needed Provide skilled nursing interventions in accordance with physician orders Maintain accurate, timely, and compliant documentation Collaborate with physicians and interdisciplinary team members to ensure continuity of care Educate patients and caregivers on treatment plans, medications, and disease management Manage a dynamic schedule and caseload efficiently while meeting productivity expectations Required Qualifications Active Registered Nurse (RN) license Strong clinical assessment and documentation skills Ability to work independently and manage a flexible schedule Experience with SOC, ROC, and recertifications Preferred Qualifications Prior home health experience Strong time management and organizational skills Excellent communication and interpersonal skills Ability to adapt to changing patient and scheduling needs Pride-Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
