Key Responsibilities\n- \n
- Conduct comprehensive assessments of clients’ psychosocial needs, integrating emotional, social, and mental health factors. \n
- Collaborate closely with clients and their families to design personalized care plans that cater to each individual's goals and preferences. \n
- Provide empathetic counseling and support, assisting clients in navigating life transitions, such as dealing with grief, changes in health, and family dynamics. \n
- Advocate for clients to ensure their access to essential community resources and services, reinforcing their right to receive proper care and support. \n
- Engage with multidisciplinary teams comprising healthcare professionals, caregivers, and other social workers to ensure comprehensive service delivery. \n

- Facilitate effective communication among family members and help resolve any conflicts that may arise regarding care decisions. \n

The primary focus of this position is the senior population, particularly those receiving hospice care. - A Master’s degree in Social Work from an accredited institution. \n
- Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of New Jersey. \n
