Position Highlights & Responsibilities

• Comprehensive Primary Care : Diagnose and treat a broad range of acute and chronic conditions, conduct physical exams, order and interpret diagnostic tests, and develop patient management plans.
• Multi-Disciplinary Teamwork : Collaborate within a Patient Aligned Care Team (PACT) including an RN care manager, LPN, clinical pharmacist, psychologists, and social workers to deliver coordinated, wrap-around care.
• Education & Mentorship : Teach and mentor medical students, residents, and other learners, fostering the next generation of clinicians.
• Integrated Support Services : Work in a system that provides seamless access to social work, travel assistance, housing, and pharmacy services-without the hassles of pre-authorizations or multiple formularies.
