We are seeking a dynamic and compassionate Primary Care Nurse Practitioner to join our dedicated healthcare team. In this vital role, you will provide comprehensive primary care services to diverse patient populations, including pediatrics, geriatrics, and individuals with complex medical needs. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions across outpatient settings. Responsibilities

Conduct thorough patient assessments, including vital signs, physical examinations, and medical histories to determine health status and formulate treatment plans.

Manage a broad spectrum of medical conditions utilizing evidence-based protocols, including acute pain management, infectious disease care, and chronic disease management.

Administer injections such as immunizations and botulinum toxin treatments; perform suturing and sterile procedures with precision.

Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner for accurate documentation of patient encounters and medical records.

Coordinate care through case management strategies, discharge planning, and working with specialists in areas such as gastroenterology, geriatrics, behavioral health, and hospice & palliative medicine.

Provide telehealth consultations to expand access to care while ensuring HIPAA compliance and secure patient communication.

Assist in diagnostic evaluations using laboratory experience, sonography, or medical imaging techniques; interpret ICD-10/ICD-9 coding for billing accuracy.

Support patients across various settings including outpatient clinics, assisted living facilities, nursing homes, hospitals (Level I & II trauma centers), ICU units, PICU, neonatal ventilator units, and home care environments.

Educate patients on health coaching topics such as nutrition, medication administration, family planning, and disease prevention strategies.

Collaborate with multidisciplinary teams to ensure optimal patient outcomes in complex cases involving dialysis, ventilator management, or catheterization procedures.

Experience

Proven experience as a Nurse Practitioner in primary care or related specialties such as internal medicine or family practice.

Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; experience with Athenahealth or eClinicalWorks is a plus.

Strong background in managing acute care scenarios including emergency room experience or hospital medicine settings.

Experience working with diverse populations including pediatrics (including toddler and neonatal care), geriatrics (memory care & dementia), behavioral health patients, and individuals with developmental disabilities.

Skilled in performing clinical procedures such as IV insertion/infusion, phlebotomy, venipuncture, spinal tap procedures, catheterization techniques (urinary & vascular), tube feeding management, and sterile processing techniques.

Knowledge of CPT coding for billing purposes; familiarity with ICD-10/ICD-9 coding standards is essential for accurate documentation.

Critical care experience from ICU or trauma center environments; Level I & II trauma center experience highly valued.

Additional qualifications include telehealth experience; expertise in infection control practices; understanding of Workers' Compensation law; hospice & palliative medicine knowledge; and proficiency in managing complex cases involving spinal taps or dermal fillers.
