Qualifications:

  1. Associates or Bachelor’s degree in Human Services, Psychology, Counseling or related field and/or two years’ experience in working with behavioral health consumers
  1. Experienced in providing trauma informed care
  1. Ability to work in cooperation with internal and external personnel and agencies
  2. Ability to work with and advocate for individuals with diverse backgrounds
  3. Is person-centered in approach and possesses strong critical thinking skills
  4. Possesses strong crisis prevention and intervention skills
  5. Certified in First Aid, CPR, and as a Medication Aide before completion of probationary period
  6. Has a valid Nebraska Driver’s License and is insurable with a good driving record
  7. Strong organizational, interpersonal, written and verbal communication skills
  8. Ability to perform efficiently and accurately in a fast-paced, deadline-oriented work environment.
  9. Ability to multi-task when necessary
  1. Possess exemplary stress management skills
  2. Possess a non-judgmental, caring attitude toward persons served
  3. Strong computer skills. Experience with electronic health records preferred

  1. Familiarity with CARF standards and a willingness to conform to all standards
  1. Ability to work full-time (a minimum of 40 hours a week) to include evenings and/or weekends as required

  1. Ability to work successfully as a team member, as well as independently

Duties and Responsibilities:

  1. Conduct Orientation activities:
    1. Complete the initial needs assessment with new admissions.
    2. Develop a case management goal plan for each residential consumer based on the consumer’s strengths, abilities, identified needs and preferences.
    3. Complete all orientation documentation and activities.
    4. Coordinate with the resident’s support network to provide information about the WELL programs, identify ways to support the resident and provide information about family days.
  2. Oversee Respite admission, ongoing daily activities, and coordinate discharge:
    1. Complete initial respite paperwork to include the Needs Assessment with new respite admission
    2. Complete Daily Respite Notes daily to justify Respite Services.
    3. Coordinate referrals with Emergency Community Support
    4. Complete Respite Discharge Summary.

  1. Provide individualized case management services:
    1. Meet with each resident weekly to assist in completion of case management goals.
    2. Document all case management activities within 24 hours of providing the service.
    3. Provide and follow up with referrals for necessary services identified by the resident and their treatment team.
    4. Ensure appropriate release of information forms are completed for each resident. Follow up with each resident to ensure currency and accuracy of releases.
    5. Assist the resident in finding creative ways to meet their individualized needs, while using their strengths, abilities and preferences.
    6. Work side by side with the resident to achieve their goals, incorporating teaching and modeling of skills.
    7. Act as a liaison and advocate with community agencies to help residents access resources needed to become self-sufficient.
    8. Promote engagement and active participation in services.
  2. Actively participate in weekly case staffing/treatment team meetings:
    1. Track and report case management activities to the treatment team.
    2. Identify and report progress and barriers for each resident.
    3. Display person-centeredness, compassion, empathy and strong customer service orientation. Support positive and constructive conversation focused on consumer strengths and abilities.
    4. Complete all weekly case management tasks assigned by treatment team.
  3. Residential responsibilities:
    1. Oversee and manage the Level System.
  1. Orient new consumers to the level and points systems upon admission.
  2. Prepare level sheets for each consumer.
  3. Communicate level progress and barriers during weekly treatment team meetings.
  4. Assist residents in navigating the requirements of the level system.
  5. Utilize a Motivational Interviewing approach to assist consumers in working through the levels.
  6. Make recommendations to the treatment team regarding the need for accommodations and creative strategies to address barriers to success.
  7. Make recommendations to the treatment team regarding individualized points, the need for points coaching, creative strategies to address barriers to success and bankruptcy guidelines.
  8. Conduct individualized points coaching as needed.
  1. Ensure that the overall environment remains positive, trauma informed and therapeutic.
  1. Whenever possible, the Case Manager will be present and involved with the residents throughout their daily activities, including meal preparation, chore completion, activities of leisure and groups, as assigned.
  2. Role model all activities and tasks that are expected of residents.
  3. Assist residents with problem solving, appropriate interactions, as well as utilizing positive coping and recovery skills.
  4. Employ established skills, including assertive communication, de-escalation techniques, crisis management, customer service, promotion of wellness and positive interventions.
  1. Maintain the health and safety of the environment, per agency policies and procedures.
  1. Maintain awareness at all times of resident’s whereabouts and activities.
  2. Enforce rules of the program to promote safety and structure through the use of the Points System.
  3. Keep facility clean to a standard acceptable by NDHHS Facility Licensure Department.
  4. Carry out fire and safety drills and complete necessary documentation accordingly. Report recommendations for improved safety practices to the Health and Safety Coordinator.
  5. Support and supervise residents in the performance of household tasks, making sure each resident understands and can complete tasks with whatever level of help necessary while still promoting the independence of the resident.
  1. Participate in treatment activities of the residents, as assigned.
  1. Transport residents to appointments, meetings, etc.
  2. Develop group curriculum and facilitate educational groups.
  3. Complete Medication Aide responsibilities ethically, according to the 5 rights and in accordance with agency policies and procedures.
  4. Report necessary medical and health concerns to staff Nurse promptly.
  5. Report mental health concerns to the resident’s Counselor promptly.
  1. Conduct transition and discharge activities:
    1. Begin discharge planning from the time that the consumer admits. Proactively work toward a strong discharge plan.
    2. Assist in developing the transition plan for each consumer.
    3. Work with the treatment team to ensure referrals are provided timely for community support services when the consumer is within 30 days of discharging from the residential program.

Probationary Period: Six months
