Work Environment

This position requires onsite work within a home care setting, where you will engage with patients in the comfort of their own homes. As a member of our Orthopedic team, you will collaborate closely with physicians and other healthcare providers to coordinate effective therapy strategies. Your working hours will run from 2 PM to 8 PM, Monday through Friday, allowing flexibility to accommodate patients' schedules and needs.

Key Responsibilities

• Manage individualized therapeutic care for a specified group of patients, ensuring seamless coordination with physicians and the patient care team.
• Conduct thorough patient assessments and develop personalized care plans that focus on rehabilitation goals, fostering patient engagement and enhancing their overall experience.
• Lead the physical therapy process across the continuum of patient care, delivering expert evaluations and interventions that align with best practice guidelines.
• Monitor and assess therapeutic, rehabilitative, and functional statuses of patients, contributing to the development of comprehensive plans of care that address unique patient needs.
• Deliver targeted at-home therapeutic interventions when necessary, applying your clinical expertise in a familiar and supportive environment.
• Educate patients, families, and caregivers on appropriate therapies, exercises, and the effective use of therapeutic aids or prosthetics to ensure continuity of care.
• Prioritize caseloads, assess daily staffing requirements, and effectively coordinate assignments or hand-offs to maintain workflow efficiency.

Patient Population / Client Focus

You will work primarily with patients recovering from orthopedic surgeries, injuries, or conditions requiring rehabilitation. These include both adult and pediatric populations, with a focus on tailoring rehabilitation strategies that meet diverse needs and enhance recovery outcomes. Your innovative treatment techniques will play a vital role in helping patients regain mobility, strength, and independence.

Qualifications

• A valid license to practice as a Physical Therapist in New York State.
• A Doctorate in Physical Therapy (DPT) is required for this position.
• Basic Cardiac Life Support (BCLS) certification is required or should be obtained within 30 days of employment.

Compensation and Benefits

We offer a highly competitive salary range of $100,000 to $120,000 annually, complemented by an extensive benefits package.
