Direct Support Associate

Mosaic Life Care seeks a compassionate Direct Support Associate to provide hands-on care and guidance to individuals in our community and social services programs. In this on-site role, you’ll assist with daily living activities, support health and wellness goals, and promote independence and inclusion. You will help individuals access community resources, attend appointments, and engage in meaningful activities. Working within Mosaic’s culture of compassion, innovation, and teamwork, you’ll collaborate with clinical staff and families, maintain accurate documentation, and ensure a safe, respectful environment while making a direct impact on community wellness.
