As a Primary Care Provider at PACE North, you will:

• Work as part of an interdisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants
• Deliver care in multiple settings:
• On-site clinic
• Participant homes
• Partner skilled nursing facilities
• Build strong relationships with participants and their families through education and support
• Practice in a flexible care model that adapts to participant needs rather than rigid systems

Requirements
