Job Description Summary Manages individualized, outcome‑oriented nursing care through the use of the nursing process. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, coordinating cross‑continuum care and assessing outcomes of interventions in accordance with system, departmental and ambulatory policies and procedures. This role provides clinically‑based, risk‑reducing care and coordination that facilitates the delivery of cost‑effective quality healthcare along the continuum. It directly interfaces with physicians, care managers, health care teams, patients and their caregivers to achieve desired patient outcomes. Excellent communication skills, both written and verbal, a commitment to coaching patients on health‑promoting behaviors, and an ability to listen and assimilate complex clinical information are required in the role. The ability to work autonomously and in teams is essential. Participation in quality improvement efforts, research and evaluation processes related to the management of patient care is integral to the role. ESSENTIAL FUNCTIONS Medication Administration Demonstrates knowledge of and follows SSH policies and procedures for administering and documenting medications. Completes medication reconciliation process following SSH policy and procedure. Demonstrates safe procedures for medication and vaccine administration. Plan of Care / Documentation / Patient Family Centered Care / Patient Experience Develops, evaluates, and updates individualized plans for patients and documents outcomes. Initiates patient outreach, identifies and documents patient/family needs and makes appropriate interventions, evaluating outcomes. Continues assessment/reassessment and identifies care needs within established nursing practice. Documents all patient care following nursing policy, departmental standards and disease processes. Assesses and documents patient response to each intervention, both pharmacological and non‑pharmacological. Seeks help to represent patients/families when they are unable to represent themselves. Raises ethical questions and concerns with the clinical team. Communicates and collaborates actively with health team members. Facilitates care across the continuum to ensure patient needs are met in the right setting and at the right time. Safety / Quality – Foster a "Culture of Safety" Verifies patient identification with two identifiers prior to procedures. Adheres to CDC hand hygiene guidelines and universal precautions. Ensures environment meets regulatory requirements. Demonstrates individual roles and responsibilities during codes or emergency preparedness. Professional Development Assumes overall responsibility for personal professional development. Obtains at least five contact hours per year in the area of practice. Practices within the legal boundaries of the MA Nurse Practice Act. Responds appropriately to feedback and recommendations for change. Demonstrates positive working relationships with interdisciplinary colleagues. Applies critical thinking to solve problems and communicate solutions effectively. Stays informed about policy and procedure changes. Technology Utilizes Outlook, Epic, HealthStream, Workday and other required software. Operates technology solutions effectively during downtime. Compliance Works within legal, regulatory and ethical standards. Complies with applicable policies and procedures. Safeguards the privacy and security of patient information. Brings potential compliance issues through the chain of command. Complies with mandatory education requirements. Patient and Family Centered Care Conveys respect for values, preferences and needs. Recognizes patients and families as important members of the team. Collaborates with patient and family in planning and evaluation. Welcomes family participation per patient preference. Age & Culture Practices age and cultural knowledge and awareness. Considers individual needs of each person. Interacts with sensitivity and inclusion. Utilizes resources such as interpreter services. Technology and Learning Participates in continued learning and adopts new technology and procedures. Embraces technological advances for effective communication. UNIT / DEPARTMENT SPECIFIC JOB FUNCTIONS Demonstrates the Aspects of Caring, including accountability and respect for patients and colleagues. Fosters positive working relationships with colleagues. Advocates for principles of positive patient experience—caring, kindness, and service. Communicates the plan of care to patients and families, keeping them informed, involving them in decisions, and using understandable terminology. JOB REQUIREMENTS Minimum Education (Preferred): Graduate of an accredited School of Nursing; BSN preferred. Minimum Work Experience: Previous ambulatory care RN experience preferred. Required Licenses / Registrations: Registered Nurse. Required Classes / Skills: BLS – Basic Life Support; basic computer skills. Required Additional Knowledge and Abilities: Demonstrated proficiency in ambulatory care nursing. Excellent verbal and written communication skills. Flexibility to adapt to changing priorities and regulations. Strong communication, interdisciplinary collaboration, creative problem‑solving, and critical‑thinking skills.
