LPN Care Coordinator

Joining a dedicated team, the fully remote LPN Care Coordinator will manage patient interactions, educate clients about palliative care, and handle administrative tasks while ensuring high-quality care for seniors with serious illnesses. Key responsibilities Educate patients about the value of palliative care and schedule appointments Document patient encounters and triage patient needs effectively Review and close completed cases daily while balancing clinical triage and patient calls Required qualifications Minimum 3+ years as an LPN Minimum 3+ years of experience reading and updating clinical notes At least 1+ years of relevant experience in a customer service call center Strong communication skills and comfort with technology Secure Wi-Fi connectivity; Spanish-speaking is a plus but not required
