In this role, you'll complete comprehensive evaluations, establish goals of care, and oversee therapy services all while delivering compassionate, patient-centered care.

Responsibilities
- Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
- Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
- Assess and document patient progress, modifying treatment plans as needed
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
- Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
- Maintain accurate documentation and adhere to all legal, ethical, and professional standards
