Pre-Approved Time Off : - Orientation Hours : 40 Facility & Patient Care Details Patient Age Groups : Adults, Geriatrics Daily Census : 95 - 110 for whole hospital Number of Visits Per Day : - Number of Rooms : - Number of Beds : - Additional Unit Information Interdisciplinary Support : IV Teams, Physical Therapy, Respiratory Services, Interpretation Services, Phlebotomy, Radiology, Social Services, Rapid Response Teams, Pharmacy, Transportation, Unit Secretary Patient Diagnoses : This RN Care Manager would work on inpatient adult units such as medical/telemetry, ICU, general med, observation unit or ED with support from the CM team on teams or in a shared office. Special Procedures/Unit Details : RN Care Manager assists patients and families to develop and fulfill a safe discharge plan to leave the hospital with the resources available to the individual patient. RN CM assists with durable medical equipment, home health, hospice, shelters and multiple types of post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. works with multiple community partners to arrange plans and secure authorization from insurance companies. This role does have Case Management Assistants who help with various tasks such as: SNF auth, transport, DME dispensing, calling pharmacy, 2nd IMM delivery, placement referrals, Medicaid long term care referrals and more. This role does not provide utilization review, but knowledge or UR helpful.Weekend coverage focuses on discharges and new assessments only, no patient progression or full assignment of cases, the team divides by units and assists each other as able. Special Equipment : - #Tier3 Travel Compliance SSN DOB RTO - 6 days total or 3 consecutive days can be auto-approved . Anything more, traveler will sign contract with pending RTO. *Anything more than 6 Days will potentially delay your offer while we run approval by the client. To minimize turn around please limit total RTO requests. Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD) MUST request all RTO / specific scheduling requests up-front during submittal. Post submittal/offer RTO may not be considered or granted. Inflexibility beyond contracted schedule and/or RTO may lead to termination of contract and system DNR. # of jobs in system we can sub - 1 across all Providence IRC facilities/regions Requires specific amount of exp. - 1 YR exp. unless specified in title/description OR license required, can submit with it pending , unless specified in job title Mile Radius Rule - 50 mile radius rule Local rule: accepted at lower BR References Required: 2 References covering 1 year (6 months for CNA s) within the last 3 years during the compliance process and prior to start. If yes, please include location and dates worked. *****Travelers may be expected to float between sister facilities nearby your contracted facility. Travelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.***** PLEASE NOTE - Must have a vehicle for this assignment! If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency.
