This position involves getting a clear understanding of each client's situation, helping determine the right course of care, and connecting clients with services and resources that can make a practical difference. You'll work closely with clients and other professionals involved in their care. The role also calls for someone who stays current with changes in the field and is comfortable bringing new ideas and approaches into their work.What You'll Be DoingMeet with clients to assess their needs and determine appropriate next stepsDevelop care plans based on each client's circumstances and goalsWork with other professionals to coordinate services when neededHelp clients and families understand available mental health and community resourcesProvide guidance and education that clients can use outside of their sessionsKeep up with current practices and apply relevant approaches to your workMaintain accurate records and follow professional and ethical requirements
