Speech Language Pathologist PRN

Speech Language Pathologist PRN PARADIGM REHAB & HEALTH CARE LLC, Keller, Texas, United States About this position Speech Language Pathologist PRN Responsibilities and Duties Perform initial therapy evaluations of each patient within 72 hours of receipt of referral. Make opportunities to teach referral sources about determining and recommending appropriate speech and hearing services. Communicate regularly with physicians, patients, families, and health care providers to modify plan of care based on patient needs. Participate in case coordination regularly with staff nurses and physicians, and document appropriately in medical record. Develop treatment plan based on evaluation. Provide appropriate clinical follow-up by reporting unfavorable signs and symptoms. Manage own schedule with no missed visits or late evaluations due to scheduling errors. Instruct other health team personnel and patient. Plan for discharge. Qualifications and Skills Current Texas Speech Language Pathologist License. Minimum 1 year clinical experience in home health. Current Certification in BSL/CPR. Valid Texas Driver's License and Automobile Insurance. Knowledge of WellSky EMR software is a plus.
