We are seeking a compassionate Medical Social Worker (MSW) to support patients and families with the emotional, social, financial, and environmental challenges that can impact health and recovery. Position Summary The Medical Social Worker provides skilled social work services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable regulations. Responsibilities Complete psychosocial assessments and identify patient and family needs Provide emotional support, counseling, and assistance with coping and life changes Help patients and families access community resources and support services Assist with referrals for financial assistance, transportation, housing, and other needs Advocate for patients and help promote safety, independence, and quality of life Participate in care planning, discharge planning, and coordination of services Communicate patient needs and progress with nurses, therapists, physicians, and other care team members Complete accurate and timely documentation in the EMR Participate in care conferences, staff meetings, and required training Maintain compliance with Medicare requirements, state regulations, and agency policies Qualifications Master's degree in Social Work (MSW) from an accredited program Current Florida social work license/credentials required for the position Minimum 1 year of healthcare social work experience preferred Home health, hospice, or medical social work experience preferred Valid driver's license, reliable transportation, and auto insurance Ability to meet all required background and compliance requirements What We Value Compassionate, patient-centered approach Strong communication and interpersonal skills Excellent assessment and resource coordination abilities Patient advocacy and problem-solving skills Ability to work independently in a home health setting Strong documentation and organizational skills Commitment to patient dignity, privacy, and confidentiality Why Join Elite Home Health?
