Job Summary

We are seeking an experienced HIM Coder Analyst II to join a healthcare team in Fort Worth. This position is ideal for a certified medical coding professional with strong ICD-10-CM and CPT-4 hospital outpatient coding experience , particularly with ambulatory surgery and observation cases.

The HIM Coder Analyst II will review medical record documentation, accurately assign diagnosis and procedure codes, abstract required information, and ensure coding meets applicable Medicare/Medicaid rules and guidelines.

Key Responsibilities

• Review and interpret patient medical records to identify diagnoses and procedures.
• Assign ICD-10-CM and CPT-4 codes accurately and to the highest level of specificity.
• Primarily code complex ambulatory surgery, special procedure, and observation cases .
• Assist with emergency department, outpatient ancillary, specialty clinic, and clinic visit coding as needed.
• Maintain a minimum expected 95% coding accuracy rate .
• Accurately abstract required information from medical records and enter data into the electronic health record system.
• Submit physician queries when clarification or additional documentation is necessary for accurate coding.
• Collaborate with Clinical Documentation Specialists and healthcare providers regarding documentation requirements and coding accuracy.
• Identify and report documentation and risk-management concerns appropriately.
• Maintain current knowledge of coding rules, guidelines, documentation requirements, and regulatory changes.
• Protect patient confidentiality and handle sensitive health information appropriately.

Required Qualifications

• RHIA, RHIT, CCS, or CPC certification required.
• Minimum 2 years of current, full-time ICD-10-CM and CPT-4 hospital outpatient coding experience .
• Experience coding ambulatory surgery and observation cases required.
• Medical record abstracting experience required.
• Strong working knowledge of electronic health record applications and automated coding/encoder systems.
• Strong understanding of applicable coding rules, policies, and procedures.
• Ability to work independently and productively with minimal supervision.
• Excellent attention to detail, organization, communication, and critical-thinking skills.
• Ability to appropriately solve coding and documentation issues using current policies and procedures.
• Must maintain confidentiality.
• Must successfully complete an on-site coding skills assessment with a minimum score of 90% prior to hire .
• Must provide current AHIMA continuing education certification records , as applicable.

Preferred Qualifications

• Pediatric coding experience highly desired.
• Experience with Microsoft Excel and Word highly desired.
