Location: Tarlani Healthcare Employment Type: Full-Time / Part-Time Per Diem Shift: Not applicable Exempt Status: Non-Exempt Job Description Summary The registered nurse plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Patient Care Responsibilities Complete an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illnesses. Regularly re-evaluate patient nursing needs. Initiate the plan of care and make necessary revisions as patient status and needs change. Use health assessment data to determine nursing diagnosis. Develop a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process. Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician. Counsel the patient and family in meeting nursing and related needs. Provide health care instructions as appropriate per assessment and plan of care. Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient. Act as case manager when assigned by Clinical Supervisor/Nursing Supervisor and coordinate patient care for assigned caseload. Communication Responsibilities Prepare clinical notes and update the primary physician at least every sixty days. Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required. Communicate with community health-related persons to coordinate the care plan. Additional Duties Participate in on-call duties as defined by the on-call policy. Ensure arrangements for equipment and other necessary items and services are available. Instruct, supervise, and evaluate home health aide care provided every two weeks. Qualifications Registered nurse with current licensure to practice professional nursing in the state. Current CPR certification. Licensed driver with a suitable automobile insured according to state/or organization requirements and in good working order. Minimum of one year visiting home health care experience. Self-directed and able to work with minimal supervision. Excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist. Ability to organize and prioritize workload independently. Responsible for supervising home health aides. Physical ability for prolonged walking or standing, lifting, positioning, transferring patients, and operating equipment; good visual acuity and hearing.
