Public Health Nurse/Nurse Case Manager
The Tuberculosis (TB) Program is seeking to fill one full-time Public Health Nurse/Nurse Case Manager. Public Health Nurse (PHN) provides case management services to patients with active or latent disease, care coordination, referral, and follow-up, contact investigation, medication management, and ongoing physical and psychosocial assessment to individuals and families. This PHN will support patients, community members and other healthcare providers with TB education and outreach and partner with healthcare providers to understand and improve TB related healthcare practices. The PHN may also participate in research projects that seek to improve the health and treatment of TB patients.
