Job Description
Oxford Nursing Home is seeking a full time Director of Social Services to join our team. The ideal candidate will have experience in a long term care or sub-acute setting and must have previous healthcare experience.
Duties include
- Manage and oversee the entire Social Service Department
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the resident’s admission and discharge to the community
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Assess residents on a regular basis
- Coordinate investigations for resident grievances
Requirements :
- Experience with MDS 3.0
- Proficient in discharge planning and memory support
- Long term care/sub-acute experience required
- Familiarity with EMR and basic computer programs
