Physical Therapist (PT) – SNF / LTC – Travel Assignment New Providence, New Jersey – Travel Physical Therapist Job Details Location: New Providence, New Jersey Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks Hours per week: 40 (8‑hour shifts) Employment Type: Travel Responsibilities Assess referred patients requiring rehabilitation services; provide direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation. Establish and administer a treatment program with specific goals tailored to the patient’s capacity and tolerance under physician direction. Set realistic and achievable goals; document and record the patient’s progress. Educate patients and families on physical therapy methods and care strategies. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduated from an accredited physical therapy program. Current state license as a Registered Physical Therapist (or actively pursuing licensure). Current CPR certification. Ability to develop a plan of care for each patient and provide skilled PT services/interventions per physician orders. Strong assessment, planning, evaluation, and modification skills to meet therapy goals. Excellent collaboration skills across multidisciplinary teams.
