For the past four years, we've partnered with top-tier nursing and skilled care facilities nationwide to manage pain and enhance therapy outcomes, helping thousands of patients regain independence and comfort.We're Hiring: We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.Day-to-Day ResponsibilitiesPrimarily responsible for regional oversight of clinical care and facility relationships, you will also round 2-3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations.Key responsibilities include:Clinical point of contact for all Providers in your regionClinical training to ensure all Providers display excellent clinical decision making and bedside mannerTime shadowing each Provider in the regionRegular clinical and facility meetings with facility staffFocused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.Supporting physiatrists on-site and operating with a high level of autonomyCoordinating care and referring to other specialties or services for issues beyond the scope of physiatryThis role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.Qualifications5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)DEA licensed or willing to obtain DEA licensureSkilled Nursing Facility experiencePhysiatry, Pain Management or Physical Medicine & Rehabilitation experienceComfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)A genuine passion for providing professional, personalized care in an autonomous environmentRelationship and value proposition building with key stakeholdersStrong communication skills, with the ability to collaborate with multidisciplinary teamsExperience in a clinical leadership or supervisory roleWillingness to travel regularly within the assigned regionWhy Us?Hands-on training in facility settings to ensure consistent, high-quality care.Stable schedule, with no weekends, nights or on-call shifts.Gain valuable experience in joint injections and rehabilitation care.Competitive salary and bonus structure (starting base of $200,000-$250,000/year depending on experience).Comprehensive benefits package: medical, dental, vision, life insurance (we cover a $50K policy with the option to add additional coverage), and AD&D insurance.18 paid PTO days, 6 paid federal holidays - 24 total paid days off.Paid malpractice insurance ($1M/$3M policy).License renewal allowance (up to $1100/year)Full autonomy in patient care with direct impact on patient outcomes at multiple facilities.Immediate Hiring Timeline: We're looking to make hiring decisions soon, with a start date in 4-6 weeks following insurance and Medicare credentialing.
