Key Responsibilities Provide nursing care and administer prescribed medications and treatments. Perform G-tube/PEG care, tracheostomy care, and suctioning as needed. Monitor patients' conditions and promptly report changes in health status. Provide ventilator support when required and appropriately trained. Educate patients and caregivers and maintain accurate clinical documentation. Qualifications Active New York State LPN license. Minimum of six months of home care experience. Current CPR/BLS certification. Strong clinical and communication skills. Compassionate, dependable, and able to work independently. Ability to lift up to 50 pounds as needed.
