LPN Care Coordinator

An “LPN Care Coordinator” coordinates the patient transition along the healthcare continuum through assessments, referrals and interdisciplinary planning for patients with chronic diseases. The LPN Care Coordinator is responsible for development, implementation & revision of the patient care plan in collaboration with the interdisciplinary team. Responsibilities Assesses the care transition needs of the patient Develop and implementation of the plan of care Revision of the care of plan as needed Develop patient centered goals with the patient and support system Collaborates with Interdisciplinary Team in development of the transition plan of care Accompanies patients to physician visits as needed Fosters an environment of individual responsibility Re‑enforces with patients the physician prescribed treatment and discharge plan Ensure documentation is complete to meet billing requirements Provide community resources as needed Secondary Function Supports the Clinical staff under the direction of qualified medical personnel within the scope of skills and abilities. REQUIRED EXPERIENCE

Six months previous clinical experience with a general understanding of higher complexity diagnosis preferred.

REQUIRED EDUCATION

Graduation from accredited Nursing Program and valid Indiana licensure

REQUIRED LICENSE

The work setting will be within the NHC practice(s).
