Care Manager & Clinical Scheduler for Proactive Care

Your Health is seeking a Clinical Scheduler to support our Care Manager role in the Ladson office, serving Charleston patients. You will coordinate all in-person facilitator visits and provider encounters for the care team, ensuring the right number and type of visits under our value-based care model. This full-time, salaried position runs 8:00 AM–5:00 PM, Monday to Friday, with responsibilities to manage scheduling across disciplines, monitor patient engagement, and use multiple software #J-18808-Ljbffr Your Health
