Coordinates management of care and ensures optimum utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment. Engages in care planning and implementation. Conducts overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care through integration of functions. Utilizes case management, utilization review, and discharge planning functions. Serves as a patient advocate through resource utilization. Addresses the holistic needs of the patient. Ensures patient preferences are taken into account.
