MDS Assessor RN

Fairview Skilled Nursing Home is seeking a detail-oriented MDS Assessor RN to complete resident assessments, ensure accurate MDS documentation, support regulatory compliance, and contribute to quality resident care. Responsibilities Complete MDS assessments accurately and on time. Gather assessment information from residents, families, and the interdisciplinary team. Ensure documentation supports MDS coding and regulatory requirements. Collaborate with nursing, therapy, social work, and other departments. Maintain compliance with CMS, Medicare, Medicaid, and PDPM guidelines. Assist with care planning and quality improvement initiatives. Qualifications Current New York State RN license. Skilled nursing or long-term care experience preferred. Knowledge of MDS 3.0, PDPM, Medicare, and Medicaid.
