SUBSTITUTE NURSE

Substitute Nurse Position
Any individual interested in becoming a substitute nurse for the Hardin Jefferson Independent School District is required to have a completed application on file prior to the district's substitute teacher orientation. A completed application includes:
- Employment application for substitute nurse
- Verification of education (official copy of a public high school transcript or college transcript)
- DPS CCH verification form
- Criminal history record information form (CHRI)
- W-4 form
- Employment eligibility form or I-9 (including a copy of your social security card and driver's license)
- Acceptable user policy agreement (AUP)
- LVN or RN
All substitute applicants must be fingerprinted. The fingerprinting process will be initiated once your application is received. Each substitute is responsible for the cost of fingerprinting.
Qualifications: A substitute applicant must have one of the following:
Pay rates: RN $120/day LVN $110/day
