Case Manager

Cardinal Hill Rehabilitation Hospital

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for inpatients recovering from complex conditions. You will assess patient needs, develop individualized discharge and care plans, collaborate with physicians, therapists, and nurses, and secure services and equipment to support safe transitions home or to the next level of care. This mission-driven, patient-centered role requires strong communication, insurance and resource navigation skills, and a passion for helping patients regain function and independence.
