Develops the plan of care provided by the home health aide, RN, and LPN by main communication with the clients/caregivers and physicians to facilitate continuity of care, prioritizing care, identifying goals, providing appropriate teaching materials and information on community resources to clients/caregivers, and coordinating services on behalf of the client/caregiver to ensure continuity of care (i.e., equipment/supplies, pharmacy, medical services, transportation, etc.) Implements the plan of care by working closely with the scheduler/s to assure appropriate caregivers are assigned, updates the plan of care based on assessments and supervisory visits, evaluates effectiveness of the plan of care at recertification, and as needed. Provides supervision of Private Duty RN, LPN, and Home Health Aides to ensure quality and continuity of care and that documentation meets the patients plan of care and needs. Reviews documentation of the supervised disciplines for appropriateness and completion compared to the plan of care. What is Required? Graduate of an accredited RN Program 2+ years’ experience in a clinical care setting or home health environment preferred Current state license as RN Excellent communication skills Dedication to patient care Ability to work as part of inter-disciplinary team You will report to the Branch Director or Clinical Manager.
