Responsibilities
- Evaluate patients' physical abilities and limitations through comprehensive assessments.
- Develop individualized treatment plans based on patient needs and goals.
- Implement therapeutic exercises and interventions to enhance mobility, strength, and function.
- Educate patients and their families on treatment plans, home exercises, and injury prevention strategies.
- Collaborate with healthcare professionals to ensure coordinated care and effective discharge planning.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Stay updated on the latest advancements in physical therapy practices and techniques.
- Provide home community care as needed to support patients' recovery in their own environments.

Experience
- Proven experience in patient care within a clinical setting is preferred.
- Knowledge of physiology, anatomy, medical terminology, and discharge planning is essential.
- Familiarity with both inpatient and outpatient physical therapy practices is advantageous.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work collaboratively in a fast-paced environment while maintaining a focus on quality care.
