Responsible for providing PT treatments to employees/patients based on an evaluation of their need and status. Maintains appropriate records and assists with in-services and Community Health Program. Mt. Ascutney will not consider any candidate who resides within 50 miles of facility. To avoid delays in processing, please provide perm address at profile upload. EMR: Cerner 8hrs non-billable orientation Holiday, Call-Back and OT multipliers: 1.25% Patient Population: Pediatric to Geriatric Dress Code: Personal Scrubs of any color Trauma Level: Critical Access Hospital

Requirements:

• 2+ years current PT Inpatient experience [Required]
• At least 6 months previous travel experience - [Highly Preferred]
• Cerner experience [Preferred]
• Traveler must have their own vehicle for this need
• Travelers perm address must be included in profile
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer/at interview. Please obtain this information prior to submittal. Requests after offer may lead to rescinded offer

• 1 Supervisory Signed Reference from within the past two years of employment [REQUIRED] [DOCUMENT]
• Active American Heart Association BLS - [REQUIRED] [DOCUMENT]
• Active Vermont Physical Therapy license - [REQUIRED] [DOCUMENT]

NOTE MT ASCUTNEY 3 STRIKE RULE FOR CALL OFFS (enforced immediately):

• After 3 call offs, a traveler's contract will be eligible for termination due to lack of attendance at Mt Ascutney Hospital and Health Center's discretion
• Each call off must be communicated to the unit supervisor at least 1.5 hours in advance
