RESPONSIBILITIES Patient Care & Service. Promote and restore patients’ health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation. Accurately and completely document client care on activity sheet Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements REQUIRED QUALIFICATIONS HHA Certified or Ability to meet one of the following eligibility requirements for Competency: Currently certified CNA along with completion of written test and one day HHA Skills competency class within 30 days of hire. Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program The Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 30 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed. Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 30 days of hire. Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 30 days of hire. PREFERRED QUALIFICATIONS High school diploma or equivalent For ElderONE employment and other areas of elderly care: 1 year experience working with the frail elderly population Compassionate, patient focused and dependable professional Critical thinking skills, decisive judgment and the ability to work with minimal supervision Exceptional documentation and communication skills Ability to follow verbal and written instructions EDUCATION LICENSES / CERTIFICATIONS HHA - Home Health Aide certification - NYS Office of Professions, NYS DL - valid New York State Driver’s License - New York State Department of Motor Vehicles (NYSDMV) PHYSICAL REQUIREMENTS Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE $18.65 - $20.00 CITY Rochester POSTAL CODE 14609 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
