Responsibilities

• Provide outpatient family medicine care to patients across the lifespan, depending on clinic need
• Evaluate, diagnose, and manage acute and chronic medical conditions
• Conduct wellness visits, preventive screenings, and follow-up care
• Manage common chronic conditions such as diabetes, hypertension, asthma, COPD, hyperlipidemia, and related primary care needs
• Provide patient education focused on prevention, medication adherence, lifestyle improvement, and disease management
• Collaborate with physicians, APPs, nurses, medical assistants, and care coordination staff
• Maintain accurate and timely documentation in the electronic medical record
• Support culturally responsive care for patients from diverse backgrounds

Qualifications

• Current, unrestricted California Nurse Practitioner license
• Family Nurse Practitioner certification preferred
• Current DEA license or eligibility to obtain
• BLS certification
• Primary care, family medicine, community health, or FQHC experience preferred
• Spanish-speaking ability strongly preferred
• Strong communication and documentation skills
• Ability to work collaboratively within an interdisciplinary care team
• Commitment to serving diverse and underserved communities
