Licensed Practical Nurse Weekend Baylor (LPN)
Night Shift
Job Summary:

Responsibilities:
Provide direct patient care, including administering medications, wound care, and vital signs monitoring, under the supervision of a registered nurse or physician
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Record patient information, including progress notes, discharge instructions, and discharge summaries
Participate in quality improvement initiatives to ensure optimal patient outcomes and care
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
Maintain accurate and timely records, including medication administration records, incident reports, and care plans
