Dual Lead

The Dual Lead coordinates and communicates with the support team to maintain, promote and monitor individuals' health and wellness. Coordinates all medical and dental appointments and aftercare. Communicates all healthcare-related information to all members of the Support Team. Reviews healthcare progress data and reports on outcomes toward goal areas. Supports culturally competent recovery-based practices and person-centered planning as a shared decision-making process.

Spectrum Community Services is a non-profit organization dedicated to providing quality support and life-enhancing opportunities for people with intellectual/developmental disabilities. This may include individuals with mental illness diagnoses.

Qualifications:

• High School Diploma / G.E.D or equivalent
• Valid and unrestricted State of Michigan Driver's License with a good driving record
• Must have reliable and ongoing transportation.
• 1 year of related experience preferred
• Must successfully complete all required background clearances
