The Valley Springs Health & Wellness Center team is looking for a Care Management Coordinator, RN to create and maintain a Chronic Care Management (CCM/PCM) program within the clinic. This role involves supporting the current nursing staff to ensure quality patient care and effective management of chronic conditions. Key Responsibilities Develop and implement a comprehensive Chronic Care Management program tailored to the needs of the clinic's patients. Collaborate with nursing staff to enhance patient care and streamline care management processes. Monitor and evaluate patient progress within the CCM program, making adjustments as necessary to improve outcomes. Provide education and support to patients and their families regarding chronic condition management. Work closely with healthcare providers to coordinate patient care and ensure continuity across services. Maintain accurate and up-to-date patient records in compliance with healthcare regulations. Participate in team meetings and contribute to the development of clinic policies and procedures related to chronic care management. Serve as a resource, offering guidance and support in managing complex patient cases.
