• Collaborating with the care team to ensure seamless patient care

• Assisting with events, celebrations, and special activities to enhance the patient experience

• Interacting with patients, families, and visitors in a warm, friendly, and professional manner

Responsibilities:

• Provide assistance with daily routines, including personal care, mobility, and transfers

• Assist with meal service, ensuring patient preferences and dietary needs are met

• Monitor and report any changes in patient condition or behavior to the care team

• Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
