As a Licensed Practical Nurse (LPN) at Xenia Health and Rehab, you will play a vital role in providing high-quality care to our residents in a warm and welcoming environment. Responsibilities:
Provide direct patient care under the supervision of a Registered Nurse (RN) or other medical professionals
Monitor and record vital signs, medical conditions, and medication administration
Assist with daily living activities, such as bathing, dressing, and grooming
Participate in shift reports and communicate effectively with other healthcare staff
Maintain accurate and up-to-date resident records and reports
Collaborate with the interdisciplinary care team to develop and implement individualized care plans
